What are Social Services?

Are you experiencing a life event that leaves you feeling stuck or like you are spinning out of control?

 

Professional Case Managers at CCI meet with you to explore all aspects of your life, focusing on areas that are going well, and assisting in the development of a plan to address areas that may need improvement.

 

Case Managers utilize strength-based, client-centered strategies, to help you recognize your individual strengths, and support your access to services and key resources on your journey to wholeness and self-sufficiency. Case Management is a process built on your individual needs and goals. This may be accomplished in a two-week period or require on-going support for several months. Either way, we will stay by your side until you feel confident, empowered, and are back on your feet.

What types of problems can you help with?

We help with many different issues, but for the most part they fall in one of these categories:

  • If you find yourself living paycheck to paycheck, struggling with debt or binge spending, or consistently calling around month to month for financial assistance, case management and our financial education at CCI might be the program for you. In all our programming we emphasize financial well being and sustainability, as this tends to be the primary source of stress for many individuals and have a resounding impact on all areas of life. If you are ready to put in the work, we're ready to empower you to take back your finances. 

  • Through our case management services we offer two primary subsidized housing programs: the Senior Subsidized Housing Program, and the Family Stabilization Housing Program for single parent families. Our housing programs are not designed to be emergency rental assistance, but rather a proactive program to prevent you from needing emergency assistance. While you are participating in our subsidized housing programs, you will have a designated case manager meeting with you weekly and assigning tasks to assist you in reducing your expenses, living within your means, reestablishing family and community connections, and achieving sustainability after your enrollment concludes. 

  • We value positive and healthy relationship with your family, children, siblings, and partners. We place a strong emphasis on multigenerational impact. If parents feel healthy and in control of their own well being, it is much easier to prioritize the well being of your children. Our case managers are versed in several types of parenting curriculum and can help you identify which may be the most beneficial for your individual needs. If there is a need we cannot meet, we will advocate for you and connect you with community resources. 

  • At CCI, we prioritize both emotional and physical health, as we believe they are strongly connected. Approximatley one in ten families in the state of Idaho struggles with food security, and one in four struggles with mental health. With counseling and case management both available at CCI, we aim to address both these needs efficiently and quickly. Our case managers can assist with connection to local food banks, free an sliding scale clinics, education in healthy cooking and meal planning, applying for public benefits, and more. 

  • There are hundreds of other nonprofits throughout Idaho, many addressing highly specific needs. It is our job as case managers to be savvy and aware of these programs and their eligibility criteria, and to be able to connect you, the client, to programs you may be eligible for. Our intake assessments target specific areas of your current circumstances to allow us the opportunity to connect you with programs you may be eligible for without even knowing it! 

Social Services Approach

Catholic Charities takes a holistic approach to working with you and your circle of support. ​CCI Case Management services focus on these STRATEGIES:

  • The sooner you identify, diagnose, and address an issue, the less impact it will have on your life. We work with you to anticipate and plan for life events to minimize disruption. 

  • We leverage your personal strengths and resources to support life's transitions and possible future critical events.  We meet you where you are and help you move forward. We develop a positive and collaborative relationship and promote community inclusion and support.

  • We believe you are capable, unique and bring strengths to address difficulties. You can be your own agent of change and we will provide guidance where necessary. We assign our resources in a way that complements your existing strengths and resources, and acknowledge and address any power imbalances. We help you identify and address social, personal, cultural and structural constraints that may impede your growth and success.

  • Our service coordinators have specialized knowledge of community resources and are able to coordinate support with them on your behalf. This strengthens the circle of support you will receive at CCI.

Phases for Social Services

1. Engage

Meet with one of our social workers to see what your needs are and whether you're qualified to receive help. Together we will identify areas of strength and concern and develop an individual service plan including goals, tasks and strategies. And yes, there will be paperwork, but don't worry, we'll guide you through it!

Together we'll build a snapshot of your household, strengths, assets, resources, and life areas you may want or may need to change. We'll identify difficulties in personal and community relationships, health, and environment. We'll talk about social, financial and environmental resources available to you.

3. Plan

Together we'll develop a plan, set priorities, and return to review it often. You will set goals, responsibilities, and time frames for completion. We'll work together to explore creative ways of achieving your desired outcomes. We'll make sure you have information relevant to your circumstances so you can make the best choices for your life.

4. Resource

Negotiation

As you move through the steps in your plan, we'll allocate resources as needed to aid in your progress. We'll help you develop an approach that you will be able to sustain once you no longer need Case Management services.

5. Implement

This is where you begin to make changes in your current environment, that will set the stage for the improvements you seek. Your plan has defined "what" you need to do, the implementation is the "doing" of the plan.

Where the monitor step applies to specific tasks, the review step addresses the complete plan including goals, strategies and outcomes. From the review you will know whether you are ready to move forward on your own, or need further assistance from your Service Coordinator.

Phases for Social Services

  • CCI’s Service Coordinator are highly skilled and educated in assessment and intervention strategies and collaborate with community partners. We have helped others who were in financial crisis, were facing eviction from their home, were food insecure, or were struggling with health challenges.  Some have struggled with personal relationships or are needing support to address their children’s behaviors.  Or some had a personal relationship that was out of control. Sometimes it just takes an objective outsider who knows what resources are available, to review your situation and provide direction. If you need services we cannot provide directly, we’ll arrange with others who can. Let’s give it a try

  • If you are unable to get to either our Pocatello or Boise offices don't stress! If you are local we are more than happy to make home visits for case management services. If you live outside our range we are well versed in remote services and happy to conduct our meetings via Microsoft Teams or phone. 

  • Please give us a call anyway! Whether you are experiencing a minor setback or a major life event, we are happy to talk with you and assess your unique circumstances. Our case management program is adaptable to your individual needs, and our case managers are able to connect you with other resources should our programming not be a good fit. 

  • Absolutely! Any information you share with us will be kept confidential, unless you authorize us to share it with a third party, or as required by law.  Your Service Coordinator will review this with you during your initial meeting.

We can meet in person or by phone/ Zoom. Once we understand your unique needs, we’ll work together to establish priorities.
