What is CASE MANAGEMENT?

Are you experiencing a life event that leaves you feeling stuck or like you are spinning out of control?
 
Professional social workers at CCI meet with you to explore all aspects of your life, focusing on areas that are going well, and assisting in the development of a plan to address areas that may need improvement. 

​Service Coordinators utilize strength-based, client-centered strategies, to help you recognize your individual strengths, and support your access to services and key resources on your journey to wholeness and self-sufficiency. 


Case Management is a process built on your individual needs and goals. This may be accomplished in a two-week period or require on-going support for several months. Either way, we will stay by your side until you feel confident, empowered, and are back on your feet.

WHAT TYPE OF PROBLEMS can you help with?

We help with many different issues, but for the most part they fall in one of these categories:
​​Financial Well Being

Are you living within your means? Struggling with debt or binge spending? We’ll help you set goals and take the necessary steps to meet those goals.

Are you currently homeless, in temporary or substandard housing, or possibly facing eviction? We’ll help you find safe and affordable housing.

​Parenting/Family Relationships

Are you experiencing violence or abuse? Having difficulty  disciplining your children, or facing separation or divorce from your spouse? Do you feel alienated from family support? We can help you develop skills to address these situations.

​Prevention and early intervention
The sooner you identify, diagnose, and address an issue, the less impact it will have on your life. We work with you to anticipate and plan for life events to minimize disruption. ​

We leverage your personal strengths and resources to support life's transitions and possible future critical events.  We meet you where you are and help you move forward. We develop a positive and collaborative relationship and promote community inclusion and support.

Strength-based practices
We believe you are capable, unique and bring strengths to address difficulties. You can be your own agent of change and we will provide guidance where necessary. We assign our resources in a way that complements your existing strengths and resources, and acknowledge and address any power imbalances. We help you identify and address social, personal, cultural and structural constraints that may impede your growth and success.

FREQUENTLY ASKED QUESTIONS

Can you really help me?
CCI’s Service Coordinator are highly skilled and educated in assessment and intervention strategies and collaborate with community partners. We have helped others who were in financial crisis, were facing eviction from their home, were food insecure, or were struggling with health challenges.  Some have struggled with personal relationships or are needing support to address their children’s behaviors.  Or some had a personal relationship that was out of control. Sometimes it just takes an objective outsider who knows what resources are available, to review your situation and provide direction. If you need services we cannot provide directly, we’ll arrange with others who can. Let’s give it a try
what if i don't have transporation to get to ONE OF OFFICES?
Services can be received in person, by phone, by videoconference (Zoom), or some combination. If you're not comfortable with technology, or don't have easy access to a computer, and live near Mountain Home or Blackfoot, we have set up a confidential kiosk at the local Parish which you may use for services.

I really only have a minor issue i need help with, is this program for me?
We offer 3 levels of service depending on your needs; together we will determine which program is best suited for you.

The first is Crisis Intervention and service lasts between 10 and 30 days. This is to help you with a short-term, immediate issues to address a single critical incident. For example a domestic violence incident, eviction or homelessness, potential disconnection of utilities, or some sort of family disruption.

The second is Moderate Level Case Management and lasts between 1 and 3 months. This is for those experiencing a single critical incident or several moderate to low level issues which will require additional support from multiple resources. Maybe someone is having difficulty finding affordable housing, and needs instruction on ways to better manage their children, we would help them address both issues.

The third is Intensive Level Case Management and service lasts 3 or more months. For clients experiencing three or more instabilities, they will be identified, prioritized, and worked in a manageable sequence. 


will discussions be confidential?
Absolutely! Any information you share with us will be kept confidential, unless you authorize us to share it with a third party, or as required by law.  Your Service Coordinator will review this with you during your initial meeting.
​HOW DO I GET STARTED?
Simply give us a call. After a brief discussion we’ll send you paperwork to complete in advance of our first meeting, or match you with a Service Coordinator to provide emergency crisis intervention support. We can meet in person or by phone/ Zoom. Once we understand your unique needs, we’ll work together to establish priorities.
